What happens before I leave the hospital?

Your Clinical Leader collaborates directly with the hospital care team to review discharge notes, medications, and recovery instructions. This helps ensure your care plan starts immediately when you arrive home, and recovery plan aligned with your physician’s recommendations.

What does the post-hospital recovery include?
