Our Privacy Commitment

Vyta is and always has been committed to ensuring the confidentiality and protection of personal information in its possession.

Vyta´s Privacy Management Program is governed by the information handling practices of the Personal Information Protection and Electronic Documents Act (PIPEDA) and applicable provincial and professional standards.

We Are Accountable

At Vyta, all employees are accountable and committed to ensuring that your personal information is held in the strictest confidence. Bayshore’s Privacy Officer is responsible for the overall compliance to the requirements of all privacy legislation and Vyta privacy practices. All our Vyta employees are accountable for maintaining the privacy and confidentiality of all information they have access to in their course of their work with us. To help ensure this, all Vyta staff are trained in our Privacy Management Program and acceptable privacy practices.

Scope

This policy relates to the personal information collected by Vyta, a line of business of Bayshore HealthCare Ltd. dba Bayshore Home Health (“Bayshore”), as well as their websites, mobile applications and banners. Throughout this policy, we collectively refer to these companies as “Vyta”, “we ”, “us” or “our”.

This policy describes how we handle and protect your personal information, specifically,

  • What information we collect and the methods of how we collect it
  • How we use, share, store and destroy your information .
  • We also explain your Privacy Choices and how you can exercise those choices.

This policy applies to you when you visit one of our websites, sign up to be a part of any of our programs, or receive our services.

How We Define Personal Information

At Vyta, we will only ask you about information that we feel is necessary for the purpose of providing the best guidance on the appropriate services for aging in place. We do not indiscriminately collect information, and constantly review our practices to ensure this remains true.

  • Your name, email address, home address and phone numbers;
  • Services you may have purchased;
  • Financial information – payment method used, transaction details including amount, type, date and time via our payment portal;
  • Sensitive information – personal preferences e.g. dietary, type of dwelling you live in, etc.;
  • Personal health information – using any of our healthcare services provided by our healthcare service partners which includes any information that identifies or can identify you and relates to the state of your health or the healthcare services you are receiving.

What We Collect and Why

We collect personal information in the following ways:

    • Directly From You – via phone, email, home visits or in any other direct manner
    • Through our websites and mobile applications –
      • we may collect certain types of information electronically when you interact with our websites and mobile applications,
      • via our online advertising, or through the use of our or a third party’s technologies, which include cookies, Web beacons and single pixel gifs and have link to their respective privacy policy
    • Our Dx Platform –
      • what actions you take on our websites and mobile applications and allows our website and mobile applications to work correctly.
    • Through Social Sharing Features –
      • your interactions with those features are governed by the privacy policies of the entities that provide these features. For more information about the purpose and scope of data collection and processing in connection with social sharing features, please visit their privacy policies.
    • Google Analytics –
      • we make use of Google Analytics to monitor and analyze the use of our website. You can opt out from the use of Google Analytics by downloading and installing the Google Analytics Opt-out Browser Add On. Google’s Privacy Policy is available here: google.com/privacy.

Your information maybe transmitted outside of Canada when using our partnering providers. We use your personal information to provide you with our programs and services, to manage our business operations, to communicate offers and information we think might interest you, to generally enhance your customer experience with us, and as permitted or required by law.

Providing guidance on the Programs and Services – use your personal information in order to provide you with our programs and services, which include:

    • Providing you with a quote for a program, or service;
    • Validating the contact information that you provide us, such as your name, email address and phone number, through service providers to be able to communicate with you about programs, or services;
    • Creating, administering, and maintaining your account;
    • Processing your transactions;
    • Fulfilling your service requests & inquiries, and purchases;

Managing Our Business – We use your personal information for many business reasons, which include:

  • Enabling your participation in surveys, chats, or online social events;
  • Conducting market research
  • Protecting parties from errors and fraud;
  • Monitoring and investigating incidents and managing claims;
  • Maintaining our programs and services;
  • Meeting our legal and regulatory obligations.

Communicating With You – We use your personal information to communicate with you in a variety of ways:

  • Responding to your inquiries;
  • Taking or verifying instructions from you;
  • Providing you with information and updates about our programs, services, promotions and events;
  • If we have your consent, informing you in a variety of ways (e.g. email, telephone, SMS,) about programs, services, special offers, or events that may be of interest to you;
  • Subject to your right to withdraw consent, providing you with offers or services based on the approximate location provided by your mobile device (also called “location-based advertising”).

Conducting Market Research & Data Analytics –

  • We may use your personal information in order to conduct market research by tracking and analyzing current collected information to improve or to develop new services, programs, promotions, or events, and to better understand our customer base.

We may use your personal information in an aggregated format or with personal identifies removed to conduct data analytics for business purposes, such as:

  • Managing, developing or improving our business and operations;
  • Improving our programs, and service offerings;
  • Understanding customer needs and preferences and customizing how we tailor and market products and services to our customers based on their interests and sentiments; and
  • Measuring the effectiveness of our marketing.

Sharing Your Information

We may share your personal information within our group of companies or with our service providers (companies operating on our behalf) for the purposes described in this policy and in accordance with applicable law  A link to our partnering service providers privacy policy is available from their website. We do not sell your personal information to any organization or person.

Please note that we may need to collect additional personal information to validate the identity and authority of the individual making the access request. This is to ensure that only individuals authorized to access your information can do so. This additional information will only be used for the purpose of validating the information request.

We will therefore share your personal information as follows:

  • In response to a request for information if we believe disclosure is in accordance with, or required by, any applicable law or legal process, including lawful requests by public authorities to meet national security or law enforcement requirements.
  • If we believe your actions are inconsistent with our user agreements or policies, if we believe you have violated the law, or to protect the rights, property, and safety of Vtya or others;
  • With your expressed consent or at your direction.

Changing your Privacy Choices

With Vyta there are several options available for you to manage your privacy preferences.

  • Via Your Vyta AccountWithin the mobile & web application, you may update your preferences via your account settings
  • Via your Vyta Expert – By contacting your Vyta Expert, you may opt-in to: receiving email or SMS communications at the time that you register for our programs, or services, or any time thereafter through your Vyta online account.
  • You may opt-out of:
    • Receiving email or SMS communications from the sender, by clicking “unsubscribe ” within any marketing email you receive, texting “STOP” within any SMS you receive;
    • Receiving surveys by following the unsubscribe instructions provided in the email;
    • Changing your browser settings on your device – Please refer to your browser instructions or help screen to learn more about how to block, delete and manage cookies on your computer or mobile device.

Our Safeguards

Vyta considers your information extremely confidential and has put many safeguards in place to ensure its security. Our security measures strive to adhere to PIPEDA requirements and appropriate provincial privacy health legislations in all aspects of the work we do for our clients and customers.

In order to keep the Personal Information secure, we restrict unauthorized access through protective policies, procedures, in addition to physical and technical measures, including:

  • Electronic information is protected by computer firewalls, user names and passwords according to assigned authorization levels.
  • Multi-Factor Authentication, which is a method of confirming your identity by using more than one way to authenticate you, such as asking for your password and a code that is sent to your email address or phone number;
  • encrypting data in transit and at rest using industry standard protocols;
  • Logging and monitoring, which is the process of tracking, recording and monitoring activity related to the access and/or use of your accounts or personal information.
  • providing reasonable physical and electronic safeguards with regard to the transmission storage and destruction of Personal Information;
  • limiting access to your Personal Information to those employees or contractors who we reasonably believe require that information to provide services to you or in order to do their jobs; and
  • governing employees and other contractors by strict standards and policies to ensure that personal information is secure and treated with the utmost care and respect.

How Long We Keep Personal Information

We will store your personal information for as long as necessary to fulfill the purposes for which it was collected, except where otherwise required or permitted by law (e.g., healthcare providers must retain patient records in accordance with their statutory retention obligations).

Your Right To Correct Your Personal Information

You may contact Vyta to request a correction of an error or omission to your personal information. This request must be in writing. As appropriate, Vtya will make the correction as soon as is reasonably possible. If a correction cannot be made,Vtya will notify you in writing as to why the correction cannot be made. Both the request for correction and our response will be retained in the record.

How Do I Request Access and Restrict access to My Personal Information?

Under Canadian privacy law, you have the right to access and restrict the personal information we hold about you, subject to any legal restrictions. Upon request, we will provide you with access to your personal information within a reasonable timeframe, in compliance with applicable laws.

It is your responsibility to provide accurate, correct and complete information. If you notice any errors in your personal information or need to update it, please let us know by contacting the Vyta Team at 1-888-898-2728.
